Simply upload the files as is and it should work from the get-go! For implementation into a design just simply copy the HTML and stick it anywhere in the page!

You can view the e-mail addresses that have been entered by going to the file.txt in the same directory as your write.php file. You can protect people from viewing it with their web browser by CHMODing the read writes to zero.
