Evaluation of double integrals by iterated integration

Evaluation of double integrals by iterated integration. This is a
simple extension of DBLQUAD to non-rectangular regions of the types

D1 = {(x,y): a < x < b, c(x) < y < d(x)},

D2 = {(x,y): a(y) < x < b(y), c < y < d}.

DQUAD uses a 1D quadrature function to evaluate the iterated integrals.
Default quadrature is QUAD. Other quadratures with the same calling
sequence are QUADL and QUADGR (File ID: #23325).

EXAMPLE: Integrate g(x,y) = (1 + x + y^2)^-2 over the unbounded
region 0 < x < y, using 1D quadrature function QUADGR.

tol = 1e-6;
